Ingrown Toenails: Why They Keep Coming Back

Ingrown toenails are one of the most common reasons people come to me, and one of the most frustrating, because the same nail tends to come back over and over. If yours has been trimmed two or three times and it keeps returning, the trim was never really the problem.
What's actually going on
Most chronic ingrown nails are caused by the shape of the nail itself. The nail plate curves more steeply than usual at the edges. As it grows out, those edges press into the skin instead of growing past it. Cutting the edge away gives relief for a few weeks. The next growth wave does the same thing.
Other things that don't help
- Cutting nails too short or rounded at the corners
- Tight shoes that squeeze the toes together
- Activities that bang the toes (running downhill, soccer, ballet)
- Picking at the corners with fingers — even briefly
What actually fixes it
For mild cases, careful trimming and shoe changes can be enough. For chronic ingrown nails, the goal is to re-train the shape of the nail itself.
